Setting

  • Home Health Care: This position involves acquiring new clients, conducting client assessments, developing personalized care plans, coordinating care services, and acting as a point of contact for clients and their families.

Hours to be Worked

  • Full-Time: Monday to Friday and weekends as needed.

Duties

  • Conduct market research to identify potential clients and market opportunities.
  • Generate leads through various channels, including cold calling, networking, and referrals.
  • Conduct consultations with prospective clients to understand their needs and explain our services.
  • Present and promote our services to potential clients in a compelling and persuasive manner.
  • Attend industry events, conferences, and networking activities to expand professional network and generate leads.
  • Stay up to date with industry trends, competitor activities, and market dynamics
  • Conduct comprehensive assessments of clients’ physical, emotional, and social needs in collaboration with RN.
  • Collaborate with clients, their families, and RN to develop personalized care plans.
  • Coordinate care services and schedules with caregivers, ensuring appropriate staffing levels and continuity of care.
  • Serve as a point of contact for clients and their families, addressing any concerns, questions, or requests promptly and effectively.
  • Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes.
  • Stay informed about changes in clients’ conditions, medical treatments, and medications and inform well necessary.
  • Adhere to all regulatory guidelines and ensure compliance with industry standards.

 

Requirement

  • Strong knowledge of home care services, healthcare systems, and medical terminology
  • Excellent assessment and care planning skills
  • Compassionate and empathetic approach to client care
  • Excellent communication and interpersonal skills
  • Ability to build and maintain effective relationships with clients, and families.
  • Knowledge of relevant regulations and compliance requirements
  • Self-motivated with a proactive and results-driven approach
  • Strong presentation skills and ability to articulate the value proposition of our services
  • Willingness to travel locally for client meetings and industry events as needed
  • High school diploma or equivalent; bachelor’s degree in a related field is preferred
