Setting
- Home Health Care: This position involves acquiring new clients, conducting client assessments, developing personalized care plans, coordinating care services, and acting as a point of contact for clients and their families.
Hours to be Worked
- Full-Time: Monday to Friday and weekends as needed.

Duties
- Conduct market research to identify potential clients and market opportunities.
- Generate leads through various channels, including cold calling, networking, and referrals.
- Conduct consultations with prospective clients to understand their needs and explain our services.
- Present and promote our services to potential clients in a compelling and persuasive manner.
- Attend industry events, conferences, and networking activities to expand professional network and generate leads.
- Stay up to date with industry trends, competitor activities, and market dynamics
- Conduct comprehensive assessments of clients’ physical, emotional, and social needs in collaboration with RN.
- Collaborate with clients, their families, and RN to develop personalized care plans.
- Coordinate care services and schedules with caregivers, ensuring appropriate staffing levels and continuity of care.
- Serve as a point of contact for clients and their families, addressing any concerns, questions, or requests promptly and effectively.
- Maintain accurate and up-to-date client records, including assessments, care plans, and progress notes.
- Stay informed about changes in clients’ conditions, medical treatments, and medications and inform well necessary.
- Adhere to all regulatory guidelines and ensure compliance with industry standards.
Requirement
- Strong knowledge of home care services, healthcare systems, and medical terminology
- Excellent assessment and care planning skills
- Compassionate and empathetic approach to client care
- Excellent communication and interpersonal skills
- Ability to build and maintain effective relationships with clients, and families.
- Knowledge of relevant regulations and compliance requirements
- Self-motivated with a proactive and results-driven approach
- Strong presentation skills and ability to articulate the value proposition of our services
- Willingness to travel locally for client meetings and industry events as needed
- High school diploma or equivalent; bachelor’s degree in a related field is preferred
