Office Hours are Monday to Friday from 9:00 a.m. to 5:00 p.m. Our phone lines are open 24 hours a day. In case we miss your call, our Voice Mail is available 24 hours a day and checked regularly so that we can promptly attend to requests for immediate assistance.

We charge a variable rate based on the type of service needed, when it is provided, and where it is delivered.

There are no costs for "non-medical" assessments, but charges may be levied for "nursing" assessments. In some situations, mileage fees apply. Costs are worked out and discussed with clients prior to commencement of service.

Help is available 24 hours a day, seven days a week to conform to clients' needs and preferences.

We service the Mercer County. If a need arises, we will strive to accommodate anyone who requires service outside of Mercer County.

We are dedicated to delivering professional, ethical, and high-quality service, with a strong focus on client satisfaction and genuine care for people. Our team brings a wealth of experience in client-centered service, case management, and caregiving. We are skilled in conducting in-home needs assessments, assisting clients in identifying their requirements, and helping them develop personalized care plans. 

     

With client permission, we also provide referrals for additional forms of assistance. Our service delivery is flexible, tailored to meet the specific needs and preferences of each client. Our employees are thoroughly screened, covered by comprehensive liability insurance, and have undergone criminal background checks. 

     

They are bonded, qualified, and above all, compassionate, dependable, and trustworthy, always respecting privacy and confidentiality. We take the worry out of hiring privately by carefully training and informing our employees. We also manage any potential issues that arise, such as scheduling conflicts, illness, or shift cancellations, by arranging substitute workers and providing ongoing supervision. Regular follow-ups ensure that our services remain effective, adjusting to the changing needs and wishes of our clients.

Certain segments of the population will find our services especially beneficial such as people who are frail; elderly; housebound; terminally, chronically, or acutely ill; physically or mentally challenged; recently discharged from hospital; family caregivers; and accident victims.

What Services We Provide

Personal Care Assistance

We provide assistance with activities of daily living (ADLs) to help clients with tasks such as bathing, grooming, dressing, and toileting. Our caregivers are trained to ensure the comfort, dignity, and well-being of our clients while respecting their privacy.

Companionship

Loneliness and social isolation can have a significant impact on an individual's well-being. Our caregivers offer companionship services to provide emotional support, engage in meaningful conversations, play games, assist with hobbies, and accompany clients on outings, promoting social interaction and overall mental well-being.

Medication Management

Our trained caregivers can assist clients with medication reminders and ensure they take their prescribed medications at the appropriate times. We emphasize the importance of medication adherence to prevent complications and promote optimal health.

Specialized Care

We understand that some individuals may have unique medical conditions or specialized care needs. Our agency offers specialized care services tailored to conditions such as dementia, Alzheimer's disease, Parkinson's disease, stroke recovery, and post-operative care. Our caregivers receive specialized training to address the specific needs associated with these conditions and deliver personalized care accordingly.

Meal Preparation and Nutrition Support

Proper nutrition is crucial for maintaining overall health and well-being. Our caregivers can assist with meal planning and preparation, taking into account dietary restrictions, preferences, and nutritional needs. We aim to ensure that our clients receive balanced and nutritious meals that meet their individual dietary requirements.

Light Housekeeping

Our caregivers provide light housekeeping services, including tasks such as laundry, changing linens, light cleaning, and organization. We strive to create a clean and comfortable living environment for our clients, ensuring their safety and well-being.

Transportation Assistance

We understand the importance of maintaining social connections and attending medical appointments. Our agency offers transportation services, accompanying clients to doctor's appointments, social events, and errands. This service ensures that our clients can remain active and engaged in the community.

Nursing Services

As Community Nursing Care is generally government funded, Registered Nurses are not routinely retained on staff. Occasionally, certain individuals may not qualify for assistance or they may wish to supplement their entitlement. In such situations, we will arrange for Private Duty Nurses to provide in-home treatment, care, support and/or education.

In Home Assessments

Non-medical Assessments are conducted to find clients' functioning capabilities, to identify their needs, and to determine what services are required to assist them attain and maintain optimum independence. Nursing Assessments are an expanded version of the Non-medical Assessment as they more thoroughly examine and identify health conditions, levels of care required, and appropriate medical interventions. If indicated, coordination of care with other health professionals can be arranged. In both situations, assessors may review subsidization possibilities, develop Care Plans, make referrals, and arrange for Service Delivery Schedules according to the clients' wishes.
